Recertification

Basic Health is required by Washington State law to periodically verify members' income and eligibility for the program. This process is called "recertification." Basic Health recertifies all members' accounts at least once a year, and those members who have been assigned I.D. numbers may be selected twice a year. Whether a member is selected once or twice a year, the same requirements apply.

When selected for recertification, members need to complete and return all forms they receive along with required supporting documentation showing proof of income and Washington State residency.

It is very important that members respond to recertification requests by the due date, or they may be disenrolled from Basic Health and will not be allowed to re-enroll for at least 12 months.

Be Sure To:

  • Complete, sign, and date the Recertification Form (139.5 KB)
  • Include all required documentation
  • Send written proof that you live in Washington State
  • Include a completed copy of the Family Income Worksheet (225.3 KB)
  • Send all items so Basic Health receives them by the due date. An envelope is enclosed for your use
  • Keep a copy of all documentation for your records

Do not send original documents. They will not be returned.
